The login is the combination of:
1. The first letter of your first name.
2. The first 3 digits of your phone number.
The password is the combination of:
1. The first letter of your last name.
3. The last 4 digits of your home phone number.
If you never submitted your home number, then use your work. If you did not submit a home or work number, enter in your mobile.
So, If your name is John Doe and your telephone number is 123.4567, then
your login is J123 and your password is D4567.
